What if my school has all of the required documents on file?

Request a letter of assurance from your school/program director clearly stating that the school has the following on file and can produce copies if requested:

  1. Documentation of a TB (PPD) test completed in the 12 months prior to the internship start date.
  2. Immunization records for measles, mumps, rubella, varicella, hepatitis B, and pertussis. A seasonal Influenza vaccine is required October through March.
  3. Documentation of a recent (within the last 2 years) criminal background check (Wisconsin Caregiver Background Check preferred)
  4. Documentation of at least an 8-panel forensic urine drug screen completed no more than 30 days prior to the internship start date.

If my school doesn’t have them on file, how do I get a copy of my immunizations?

  • You can contact the doctor’s office or health center where you received the immunizations and request a copy.
  • You may be able to download the information from the Wisconsin Immunization Registry (WIR) at : https://www.dhs.wisconsin.gov/immunization/wir.htm

Where do I get any immunizations I might need to be in compliance?

  • Check with the student health center at your college or university
  • Contact the nearest public health office
  • Contact your personal physician or other healthcare provider

  • Check with the student health center at your college or university
  • Contact the Occupational Health Department at Watertown Regional Medical Center. Please note, payment is due at time of service.
